Our services are provided under the Management Services Agreement (MSA), and we work closely with you to design a custom-fit concierge or a value-add program.

Who is a good fit for the Fit in Your GENES® program?

Our program can be a value-add into an insurance-based or a fee-based practice, such as a concierge or direct pay. We see our program as Concierge 2.0, bringing new services and comprehensive care opportunities to specialty practices, irrespective of size and location.

Physicians pursue affiliation for a variety of reasons. Many are aware that their patients are looking for integrative and lifestyle medicine services, and may have engaged providers outside of the physician practice. Physicians are also looking to increase their practice flexibility while maintaining or increasing their income. Our programs offer longer appointments to patients, and opportunities for increased revenue from existing patients, new patients, and ancillary product sales, such as supplements.

Our program is data-driven and evidence-based. Each affiliated practice has transparency and monthly KPIs, as well as outcome reporting. We foster physician thought leadership and partnerships with digital healthcare companies and leading vendors in the health and wellness space.

Let’s chat! After meeting with you for the Holistic Heart Centers overview, we will determine if your practice is a strong candidate to ensure a mutual fit. We are looking for physicians in established practice, and operating in a physical location, with ties to local communities. We can also partner with virtual-only practices. We provide you with a detailed strategic and financial plan for your practice. Once you are officially onboard, we partner with you on the implementation plan, including patient and staff onboarding, vendor and lab access, supplement setup, marketing, and education for your patients and peers.

Your patients continue to be your patients, and you are responsible for all clinical care decisions. The Fit in Your GENES® program must be provided under physician supervision.

Patients can use their insurance for all services your practice provides, which may include the Fit in Your GENES® program. During the practice setup, we will align the program revenue structure for the best fit in your practice.

The Fit in Your GENES® program fee covers structured series of visits with physician and non-physician team members who provide specialized protocols, education, and communication as well as care coordination. Most of our practices choose to implement the Fit in Your GENES® program as a fee-based signature concierge offering. HeartWell toolkit fees may be bundled into the program fees.

Our program is delivered over a 3, 6 or 12-month period, and patients who graduate from the program are offered a membership option. Unlike older concierge or even newer direct pay models, this is not a one-size-fits-all monthly membership. We do not charge for same-day or enhanced access, or virtual care options as those are included. By providing access to integrative precision medicine services, we differentiate our program and enhance practice value.
